The ideal candidates
Successful individuals must have at least three years' experience working as a case manager and be a registered HCPC/NMC health professional. Located in the north, you will need clinical experience of working with individuals who have suffered limb loss and complex orthopaedic injuries is also crucial. Key responsibilities and requirements will include:
- Assess client needs and develop tailored case management plans.
- Coordinate multidisciplinary teams, ensuring effective rehabilitation pathways.
- Liaise with medical, legal, and social care professionals.
- Provide ongoing support and advocacy for clients.
- Travel across the north to conduct assessments and attend case conferences.
- Strong communication, organisation, and problem-solving skills.
- Ability to work independently and manage a varied caseload
Salary can be negotiated and will be competitive and a reflection of your experience and suitability for the role.
The roles
As an Associate Case Manager, you will be supporting the delivery of our case management services in the north of England. The ideal candidates will cover the following areas:
- Northumberland
- Tyne and Wear
- County Durham
- Teesside
- Cumbria
- Lancashire
- Greater Manchester
- Merseyside
- Cheshire
- Yorkshire.
These roles will serve key northern cities including, Newcastle, Leeds, York, Manchester, Liverpool, and Sheffield.
