We are thrilled to welcome Clair Jones to our team as an Associate Case Manager, bringing extensive clinical expertise to strengthen our services across the north of England.
COTA Case Management are delighted to be welcoming a new Associate Case Manager to the team. Located in Leeds, Clair Jones will be supporting the delivery of our specialist services across the north of England.
Having worked in the NHS for 26 years as an Occupational Therapist, Clair brings an incredible wealth of knowledge to the team at COTA. Over the course of her career within the National Health Service, she has supported a range of individuals in both clinical and community settings. Her extensive experience includes working with adults and children who have sustained amputations, spinal injuries, complex orthopaedic trauma, and neurological conditions, including brain injury.
In addition to this, Clair has specific expertise in the rehabilitation of hand injuries and upper limb conditions. Her skills in prescribing bespoke assistive equipment, conducting moving and handling risk assessments, and overseeing complex care packages reflect her comprehensive approach to client rehabilitation.
Delivering Bespoke Case Management Support in the North
One of the key reasons we’re so excited about Clair’s arrival is due to her expertise and passion for client care. With her specialist background of providing tailored support to amputees, her comprehensive expertise matches perfectly with our bespoke service offering. As we're the only case management company in the UK specialising in amputations, meaning we’re uniquely placed to offer tailored support following an amputation or complex orthopaedic trauma.
Throughout the course of her career, Clair’s worked dynamically and collaboratively with multidisciplinary teams, preventing hospital admissions and optimising functional outcomes while reducing risk for individuals with complex conditions. This has helped shape her approach to her work as a case manager, as she’s able to provide practical and empathetic solutions for clients.
We work alongside an individual, their loved ones, and representatives to create bespoke and tailored plans that truly reflect their needs and targets. We’re committed to delivering an exceptional service for our clients, ensuring that during their rehabilitation, they are empowered to rebuild their lives and reach their fullest potential following a significant injury.
Since leaving the NHS in 2020, Clair has become an experienced case manager working with Corporé, Thrive Rehabilitation Services, 3HUB and now COTA. We’re really excited about having her on board as she continues to deliver exceptional support as a case manager.
Clair’s vast clinical expertise, commitment to client-centred practice, and dedication to achieving meaningful outcomes for clients make her a valuable addition to our team.
