At COTA, we specialise in providing exceptional rehabilitation case management services tailored to individuals recovering from serious personal injuries, particularly those affected by amputation and complex orthopaedic trauma. Our highly skilled health professionals have extensive experience conducting Immediate Needs Assessments (INA) under the Rehabilitation Code 2015.
During the INA process, our team meticulously assess the individual's rehabilitation needs, considering physical, psychological, cognitive, and emotional aspects. This comprehensive evaluation informs a detailed report outlining the costs of the support and services required to maximise the individual's functional capacity.
Our professionals carry out the INAs in the injured person’s home, hospital, or other suitable environment, depending on the individual’s needs. They will fully assess the nature of their injuries from a physical, psychological, cognitive, and emotional perspective, helping to accurately determine their immediate needs for rehabilitation. From treatment and care to equipment, adaptations and accommodation, we develop a structured plan focussed on individualised goal setting to ensure measurable clinical effectiveness.
Our Referral Process:
- Enquiries can be made via email, telephone, or through our website's contact page.
- Upon review of the individual's needs, we will propose suitable case managers based on expertise, clinical experience, geographical location, and overall fit.
- Within 48 hours, you'll receive CVs, along with a quote and terms of business.
- Should further discussion be required, our managing director is available for consultation. We offer options for arranging a 'Meet and Greet' session, tailored to your client's needs.
- Once a case manager is agreed upon, they will promptly contact the individual within two working days to schedule an initial appointment.
- The injured person and/or suitable representative will receive information about COTA Case Management to ensure they fully understand the INA (Immediate Needs Assessment) process. They will also receive a consent form, enabling the release of appropriate medical information and the sharing of information with appropriate parties. This consent applies to both the INA process and any further case management interventions, should this be authorised.
- The case manager will the conduct the assessment within ten working days and deliver a comprehensive report.
- This report will include outcome measures, predictions, and costs and will be prepared and sent with ten working days of the assessment. These timescales are flexible, and should an urgent request be made, it would be recommended that this be discussed and agreed upon as part of the referral enquiry.
