Associate Case Manager Role Based In The South East– Join the COTA Case Management Team

Due to our continued growth, we are currently looking for an Associate Case Manager based in the South East of England to join COTA Case Management.

The Role

As an Associate Case Manager, you will be supporting the delivery of our case management services throughout the southeast of England. As part of this, the ideal candidate will need to be able to travel across key areas, including:

  • Berkshire
  • Buckinghamshire
  • East Sussex
  • Hampshire
  • Isle of Wight
  • Kent
  • Oxfordshire
  • Surrey
  • West Sussex.

A successful individual must have at least three years experience working as a case manager and be a registered HCPC/NMC health professional. Clinical experience of working with individuals who have suffered limb loss and complex orthopaedic injuries is also crucial. Key responsibilities and requirements will include:

  • Assess client needs and develop tailored case management plans.
  • Coordinate multidisciplinary teams, ensuring effective rehabilitation pathways.
  • Liaise with medical, legal, and social care professionals.
  • Provide ongoing support and advocacy for clients.
  • Travel across the Southeast to conduct assessments and attend case conferences.
  • Strong communication, organisation, and problem-solving skills.
  • Ability to work independently and manage a varied caseload.

Salary can be negotiated and will be competitive and a reflection of your experience and suitability for the role.
